WAIVER AND QUITCLAIM

I hereby understand and acknowledge that the training, programs and events held by BURN ROOM DUMAGUETE may expose me to many inherent risks, including accidents, injury, illness, or even death.

I assume all risk of injuries associated with participation including, but not limited to, falls, contact with other members, undisclosed clinical condition, the effects of the weather, including high heat and/or humidity, and all other such risks being known and appreciated by me.

I hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with my participation in any activity.

I acknowledge that I am physically fit and mentally capable of performing the physical activity I choose to participate in.

After having read this waiver and knowing these facts, and in consideration of acceptance of my voluntary participation, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE, BURN ROOM DUMAGUETE, its owners, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, and I hereby QUITCLAIM any claims, damages, or actions arising out of my participation in BURN ROOM DUMAGUETE’s training, programs and/or events.

I further affirm and confirm that I have fully read and will comply with all DOS and DONTS as well as GYM ETIQUETTE promulgated by BURN ROOM DUMAGUETE and/or will comply with other rules and regulations which the latter may impose for members to observe. Failing so, I acknowledge, confirm, and ratify that BURN ROOM DUMAGUETE has the right to forfeit my gym membership, without need of refund, and without prejudice to the filing by BURN ROOM DUMAGUETE’s owners, or representative(s) of the appropriate civil or criminal actions as the circumstance(s) warrant.

By clicking below, I hereby indicate that I have read and understand this Waiver of Liability and Quitclaim, and that I fully accept all the terms and conditions hereto provided, this being a prerequisite of my gym membership.

Finally, I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms as well as its binding force, without need of executing a notarial document.
